Hi there! Are you an employer new to APHT looking to purchase one of APHT’s plans? There are a few eligibility requirements you should know about first.
An employer must meet ALL of the following eligibility requirements to be eligible for APHT’s plans:
- Be a licensed physician in the state of Louisiana who is in good standing
- Be a member in good standing with the Louisiana State Medical Society (LSMS)
- The employer maintains business licensure and all appropriate state filings allowing the company to actively conduct business in the state of Louisiana
- Employers are required to have a minimum of 2 employees enrolled to qualify.
